EXCESSIVE VERTICAL DIMENSION of OCCLUDING.

Excessive vertical dimension of the DE in excursions or protrusion can cause joint stain/load, while centered clenching will present asymptomatically.




POSTERIOR CONTACT IN EXCURSIVE POSITION

Shorty following delivery, a patient may report that their incisors that oppose the NTI are suddenly sore.  This is typically due to a posterior contact that was initially unappreciated at the initial delivery, or has been created by the repositioning of the condyle.  The occurrence of a posterior contact (typically in an excursive position), allows for pathologic clenching intensity, which traumatizes the simultaneously occluding incisors.  INCREASING THE VDO OF THE DE IS CONTRAINDICATED, as that would increase the rotation of the condyle.  The preferred therapeutic remedy is to occlusally adjust the contacting molars.  (best accomplished with the NTI is place).


LOWER CANINE CONTACT ON AN UPPER DE

One of the primary advantages of the Lower Device is the avoidance of the possibility of a lower canine contacting the DE of an Upper Device in extreme excursive (shown on the left).  When providing an Upper Device, confirm the presence or absence the patient's ability to clench on the DE with a lower canine.  If new symptoms appear later, or the original symptoms intensity, switch to a Lower Device.
